Pastors Report Form

If you have trouble saving or printing a FILLED/COMPLETED copy of the Pastor's Report, click on the following link to install the latest version of Adobe Acrobat Reader DC: http://get.adobe.com/reader/       

Note: Uncheck "Optional Offers" in gray area before clicking "Install now" button.

The Pastor's Annual Report is a fillable PDF form that must be downloaded and saved to your personal computer prior to filling in your information.

  1. Click the download button located in the top right side of the PDF viewer (see Green Arrow);

  2. Click "Save" to save file in Downloads folder; 

  3. After file has been saved, click "Open" then, select Adobe Acrobat Reader DC;

  4. Once the file opens, fill in your information and save the completed form on your "Desktop" or in "Documents" folder.

  5. Now, the form is ready to be emailed as an attachment, and printed if a copy is needed for filing.
